After installing and configuring, the subscribe button published from the module doesn't do anything.

Using firebug, I discovered the following error when clicking Subscribe -

document.mcsignupForm1134.EMAIL is undefined

Has anyone seen this before?

When configuring the module you must select at least 'Email address' from the fields dropdown list. Have you done that?
